By signing below, all authors acknowledge that they have read the Criteria for Authorship and the Copyright Transfer statements. In addition, your signature affirms that this work has not been previously published, is not subject to copyright or other rights except your own to be transferred to The American Journal of Managed Care®/The American Journal of Accountable Care®/Clinical Care Targeted Communications Group, LLC, and has not otherwise been submitted for publication, except under circumstances communicated to the Editors in writing at the time the work was first submitted. Criteria for Authorship


Each author should meet the criteria for authorship as described in the “Uniform Requirements for Manuscripts Sub mitted to Biomedical Journals.” These criteria are outlined below and each author must indicate their specific contributions to the preparation of this manuscript by completing sections A, B, C, and D.

  • the manuscript submitted under my name represents valid, original work and has not been submitted elsewhere and will not be submitted for publication elsewhere while under consideration by The American Journal of Managed Care® or The American Journal of Accountable Care®; and

In the event that AJMC® or AJAC publishes my work and in consideration of the editing and publication of my work and the professional benefits relating hereto, I transfer to Clinical Care Targeted Communications Group, LLC, copyright ownership and all worldwide right, title, and interest to all parts of the work including, but not limited to: (1) copyright (text and graphics); (2) the right to grant permission to republish the work in whole or in part, with or without fee; (3) the right to produce preprints or reprints for sale or free distribution; (4) world translation rights; and (5) the right to republish the work in a collection of articles or in any format including, but not limited to, electronic media.  Authors of manuscripts reporting research funded by NIH are granted permission to provide a copy of their accepted manuscript to the NIH for inclusion in PubMed Central.
